چکیده
RESEARCH DESIGN AND METHODSdWe performed a frequently sampled intravenous glucose tolerance test (FSIGT) withminimal model analysis and ameal tolerance test (MTT) in 20 subjects with impaired fasting glucose (11 men, 9 women; mean age 60.76 1.9 years, BMI 29.4 6 0.9 kg/m) in a single-blind study after 2 weeks of placebo treatment and 8 weeks of colesevelam 3.75 g daily. From these tests, insulin sensitivity, b-cell function, and glucose tolerance were determined, along with gastrointestinal peptide levels during the MTT.

Colesevelam Improves Oral but Not Intravenous Glucose Tolerance by a Mechanism Independent of Insulin Sensitivity and β-Cell Function
OBJECTIVE To determine the mechanism by which the bile acid sequestrant colesevelam improves glycemic control. RESEARCH DESIGN AND METHODS We performed a frequently sampled intravenous glucose tolerance test (FSIGT) with minimal model analysis and a meal tolerance test (MTT) in 20 subjects with impaired fasting glucose (11 men, 9 women; mean age 60.7 ± 1.9 years, BMI 29.4 ± 0.9 kg/m(2)) in a ...
متن کاملAtypical Mechanism of Glucose Modulation by Colesevelam in Patients with Type 2 Diabetes
Colesevelam's glucose-lowering mechanism of action is not completely understood. Clinical trials of colesevelam suggest that its mechanism, and often adverse effects, differ from those of other oral antidiabetes drugs. Colesevelam does not affect insulin sensitivity (unlike thiazolidinediones), insulin secretion (unlike sulfonylureas and meglitinides), or early insulin response or glucagon (unl...
متن کاملAssessment of beta-cell function in humans, simultaneously with insulin sensitivity and hepatic extraction, from intravenous and oral glucose tests.
Assessment of insulin secretion in humans under physiological conditions has been a challenge because of its complex interplay with insulin action and hepatic insulin extraction. The possibility of simultaneously assessing beta-cell function, insulin sensitivity, and hepatic insulin extraction under physiological conditions using a simple protocol is appealing, since it has the potential to pro...

We thank Dr. Pontiroli, and agree with his commentary (1). Dr. Pontiroli provides an interesting observation regarding change in b-cell function following weight loss by gastric banding in subjects with type 2 diabetes. His data show minimal improvement in insulin release, as assessed by the Dinsulin/Dglucose during an oral glucose tolerance test (OGTT), but a significant improvement in insulin...
متن کاملExenatide Treatment Alone Improves β-Cell Function in a Canine Model of Pre-Diabetes
BACKGROUND Exenatide's effects on glucose metabolism have been studied extensively in diabetes but not in pre-diabetes. OBJECTIVE We examined the chronic effects of exenatide alone on glucose metabolism in pre-diabetic canines. DESIGN AND METHODS After 10 weeks of high-fat diet (HFD), adult dogs received one injection of streptozotocin (STZ, 18.5 mg/kg). After induction of pre-diabetes, whi...
